周手術期看護に関する研究
研究概要 【1】近年、医学特に医用工学の進歩により皮膚に直径数cmに開けた穴から、カメラと柄の長い手術器具を挿入し、従来の手術よりはるかに小さな侵襲で手術を行うことが可能となってきた(鏡視下外科手術)。鏡視下手術は細いカメラから良い手術視野を得るためにしばしば特殊な体位(姿勢)を取ることが必要となり。その成否は看護師の技量によると言っても過言ではない。一方、鏡視下手術は全身麻酔で行われるため患者は特殊体位に起因する体の所々の痛みやしびれを訴えることができず、手術自体が成功しても術後の神経障害・皮膚障害が時に発生し医療安全上問題となる。鏡視下外科手術の中で、胃がん摘出術・子宮がん摘出術など頻度の多い手術では砕石位という特殊体位が取られる。その際に下肢を支えるために使用される装具によって長時間下肢の神経(腓骨神経)が圧迫され、偶発的に下肢麻痺が起こることが知られている。現在、体圧分布測定装置を使って砕石位における腓骨神経走行部位への圧迫の程度を検討している。
【2】人工呼吸器など医療工学の発達にも関わらず、ショックや敗血症などの全身性炎症にともなって発症する急性肺傷害の死亡率は依然として高い。しかし、急性肺傷害に対する効果的な薬物療法は未だない。一酸化炭素は高濃度では人体にとって猛毒であるが、極低濃度では抗炎症・抗アポトーシス作用を介して生体保護的に働くことが近年報告されてきた。そこで、ラットのショック蘇生による急性肺傷害モデルを確立し、このモデルに極低濃度の一酸化炭素を吸入させると、炎症を抑制し肺細胞のアポトーシスを減らすことによって治療効果があることを明らかにした。現在、臨床応用を目指してさらに研究を進めている。
